The SHIP program provides support as the family and individual re-adjusts to the trial and tribulations of maintaining permanent housing, ensuring greater success either by
transitioning from the homelessness to community living, or maintaining current
housing or securing alternate permanent housing and avoiding possible homelessness.
Services include but are not limited to the following counseling and community support:
- Life Skills Training
- Budgeting/Money Management Instruction/Debt Collection Intervention
- Nutrition/Meal Planning
- Landlord/Tenant Rights
- Parenting Skills
- Child Care Information
- Employment/Education Opportunities
- Housing Assistance
- Case Management Advocacy with accessing financial assistance through Social Services (e.g. public assistance, food stamps, Medicaid, HEAP, and other emergency benefits) and Social Security Administration (i.e. SSI and/or SSD).
- Other community agency referrals such as; WIC, food pantries, household items, clothing, etc.
Eligibility Criteria:
- TANF Eligible Families who are at-risk of homelessness or homeless, in Fulton & Montgomery Counties, due to pending eviction, Niagara Mohawk termination, or other issues related to losing current housing.
- Individuals between 18-25 years of age, who have aged out of the foster care system and are homeless.
Also, we provide assistance with securing alternate housing, if current housing is inadequate or unaffordable which includes:
- Liaison assistance with landlords, Code Enforcement, etc.
- Housing search
- Procuring lease agreements
- Securing necessary financial assistance for rental costs and/or security deposits.
The SHIP Case Manager will meet with participants and provide support services for (9) months. Assistance will be provided in developing short and long term goals in working towards self-sufficiency and maintaining permanent housing.
